Perspectives on Homelessness and Integrated Care in Canada and Japan

October 12, 2026

Due to declining birth rates, an aging population, population decrease, and transformations in families and local communities, building a comprehensive support system that spans welfare, medical care, nursing care, health, housing, and employment has become a global challenge. In Japan, based on the community-based integrated care system, institutional reforms such as the Multi-layered Support System Development Project are underway to realize a "Community-Based Inclusive Society." In building a comprehensive support system that allows people to continue living safely in their familiar communities, practices involving collaboration among multiple professions, multiple agencies, and local residents are also being developed in the Pacific Rim countries under the philosophies of Integrated Care and Community Care. Addressing increasingly complex and multifaceted life challenges requires comprehensive support that transcends institutional boundaries and the expertise of social work.

This seminar will share integrated care and community-based care initiatives in Japan, South Korea, Canada, and Australia, comparing not only the institutional designs but also the characteristics of their practices. It will discuss, from an international perspective, the ideal form of comprehensive support systems, community participation, multi-professional and multi-agency collaboration, and the role of social work in supporting community development. By sharing experiences from each country, the seminar aims to explore directions for sustainable community-based integrated care.
